ABWE MKs: G.R.A.C.E. Needs Your Medical Records ASAP

MK Friends –

The G.R.A.C.E. investigative team would like to take a look at our medical histories from our years in Bangladesh.  They need us to fill out our HIPPA forms (attached) and send them in to the ABWE home office where our files are currently located.  Once they receive that form they will release our files to G.R.A.C.E. for review.  If you prefer, you may grant Dr. Joe DeCook permission to review the files on your behalf.  Otherwise, the team from G.R.A.C.E. will review them carefully.  Your information will be kept confidential either way.

Your request for your medical file must be made as soon as possible so that the investigation can move forward and be completed in a timely manner.  Please fill out the attached form and send it to ABWE as soon as you can.  Thank you!

We are a group of American former missionary kids (MKs) who lived in Bangladesh while Donn Ketcham worked as a missionary doctor there with the mission agency Association of Baptists for World Evangelism (ABWE) out of Harrisburg, Pennsylvania.
